Reporting to the RN Lead:
The role of the nocturnal RN care manager includes ongoing patient care, education, triage of condition change, and escalation to appropriate treating provider. The RN care manager is the primary care contact for incoming alert management, inbound patients communication for clinical concerns and to obtain resources or services as needed in a telehealth environment.

2 years of clinical care in a critical care setting
2 years of telephonic triage experience
Preferred experience in cardiac care and/or diabetes management
Graduated from an accredited registered nursing program
Ability to obtain multi-state licensure and maintain associated education
6 months of experience in telephonic triage and patient education

Perform three rotating 12 hour shifts per week assigned in rotation from the following shifts:
M-F 8pm-8am EST OR Sat, Sun 8am-8pm EST OR Sat, Sun 8pm-8am EST
Exceptional critical thinking skills that allow for an autonomous practice with knowledge of appropriate escalation to higher level of skill
Able to use multiple technology systems with superb phone and communication skills
Able to document according to general standards and follow care protocols
Highly skilled in telephonic assessment, following decision support tool guidelines, and triage independently including escalation to treating provider
Efficiently communicates to team members regarding patient concerns and care plans
Develop care plans that can be completed by multiple levels of skilled staff
Create and follow clinical care plans customized to patient education level, social determinants, with patient centered directed goals.
Able to follow and initiate standing orders, as appropriate
Understands and can troubleshoot technology for telehealth and remote monitoring systems

Act as first line clinical support for incoming patient concerns during after hours
Receive incoming calls from patients and follow standard triage protocols to address the patients needs.
Understands condition management, associated assessments, and signs of worsening
Manages triage for patients during after hours and escalates to treating provider per protocol
Able to assess physiological data and act on concerning information preemptively
Present patient concern to MD/NP/PA in a standard and professional format
Able to follow protocols and deviate where needed and documenting rational
Complete audits of documentation and protocol compliance for all care team members
Escalates concerns to Nocturnal manager related to patient safety and consistent deviations from protocols
Perform Quality Review for the administrative team
Perform asynchronous reviews of patient records to identify care gaps for the daytime team to address
Review physiologic data trends asynchronously and send reports for the daytime team to address
Manages dashboard of alerts patients as an escalation point for changes in condition or higher level care needs, responding to alerts within a 15 minute window, across programs
Assist with the sending of care reports to appropriate providers
Complete Post-Discharge Medication Reconciliation on weekend day shifts
Assist with all quality review and assurance activities as directed by Team Lead
Complete additional tasks as assigned by the Team Lead/Division Manager

Licensed in Texas
Bilingual in English and Spanish Language preferred
2+ years experience in cardiac care
Multi State RN license
Willingness to obtain endorsements to other states as determined by company need
